Directions:

1. Fill out the first page of this application as thoroughly as possible.
2. Read and understand all pages regarding terms and conditions, insurance, and your new account.
3. Sign where indicated.
4. Fax all pages to the Rental Department. (Number listed below).
5. The Rental Department will notify you on the status of your application either by mail or phone call.

(Please remember, it will take approximately 2 weeks to open an account).
